Read the documents

Open each one and read it. You’ll tick that you’ve read them in step 2. Bus aides also read the Bus Aide Instructions.

Give us your details and sign

One form. It sets you up for pay and super, and records that you’ve read the documents.

Have these ready:

  • Bank BSB and account number
  • Tax file number 9 numbers
  • Super fund name, USI and member number
  • Emergency contact name and mobile
  • Your address where you live now
  • Photos of your licence and WWC card

Your details go straight to the Plum Oak office in a private system, not by email. They are only used for pays, super, the ATO and emergencies. Changed bank account, address or phone? Just fill the form in again.
